Here's my quick version of how i would play it.. Hope it helps out anyone who was looking to play this song without having to re-tune the guitar. I also called the C#m chord a Cm around the 1:55 mark. My bad.. hopefully that doesn't throw anyone off... i call it correctly the rest of the time. Enjoy!
