Dan Meyers: actually, although the term used for the removal of teeth is "extraction", teeth *can* be extruded, as well, with or without tools -- extrusion can mean the pressing of a material through something else *OR* the *pulling* of a material from/through something else, as exampled for instance with high-cast metal work where steel, etc., is pulled through/out of its die-cast molding or when teeth extrude via congenital defect, accident or disease. Also, extrusion of teeth in a metaphysical context meant the loss of a facet of one's soul relative to the the concept of losing one's "face" (the bearing of one's mortal identity) and "name"(the inherent essence of identity and spirit), both of which are brought up in the song.
