I thought that the narrator was the killer, but then I realized that Joy became suicidal and depressed after a while. The narrator suggests clearly that 'And so I've left my home, I drift from land to land, I am upon your step and you are a family man....-....Do you reckon me a friend?' He asks him if he recognizes him. So I'm guessing that the killer is the family man that Nick's talking to.

No offence but that sound at the end is probably the ONLY part of the song that doesn't clearly indicate that the narrator is the killer.

I agree, of course it isn't real.. That whole album he made is about murder (hence Murder Ballads). I seriously doubt Nick Cave has done all those things he sings about :)
