Billy is the milkman. Think about it; the first thing he can find is a bottle of milk, and the little brother comes in having found Billy's hat and alcohol in his truck. The end says "if you must know the truth about the tale, go and ask the milkman." The song ended with the family heading to Tennessee. Meaning that the milkman has to be one of them (i.e. Billy) as he knows the end of the story.

preacher is the dad. boyfriend is the boyfriend. brother is the brother. Billy is the protagonist. The milkman was having an affair with the mom as well as the boyfriend. The little brother came in with the milkman's hat because the milkman left it there when he was "delivering milk" to his mom. The milkman is the other side of the story but he isn't present at the time.
