this song isnt about being over weight.its about emotional baggage.she sing "grieving of a loss sorry to be heavy but heavy is the cost" she also sings "when you see my face i hope that you dont laugh i'm not a filmstar beauty i'll send a photograph" she sings that because filmstar always smiles. and when they meet he saw that she is upset and he doesnt want to get involve with that. then in the end he sings "is it you or me?" repeatedly so im guessing he wasnt ready for a relationship
