I always thought this song was about a chick getting pregnant from her first lover (or possibly an early rapist, since she fell "in fear" upon her back, though with first relationships a lot of things can cause fear, such as having sex for the first time); wanting to leave him, possibly due to the fear ("don't look back, just keep on walking"); getting an abortion in the "early hours" ("cut some cord"); and the regret and fantasies over "what might have been" haunting her to the point that she's unable to hide her own emptiness, left heartless and unable to love again by the incident because no real man could match what she imagined her first lover to be.

See, there's the think with songs that sound like they have a meaning in them, but said meaning is difficult to determine; everyone comes up with their own idea of what the song is about, and think that the only and obvious answer.
