They didn't change her voice, she did. She is meant to sound like that, her vocals sounding like that shows the true nature of what she is singing about she sings these songs to show the true nature of the world. If she didn't do this with her vocals then there would be little point to the songs. As she has said she doesn't sing these to show happiness as a cover of the truth, she sings these to show how harsh the world really is. :) The screaming is showing the emotion in her words.
