The line "Watashi no oroka na yamai wa, Dandan hidoku natte iku bakari" means "My pathetic illness keeps exacerbating" exaberating pretty much means its getting worse. The line about shattered pieces of heart indicates that she can't do anything about her illness, and it's heartbreaking to her. I don't think the weeds growing on her tongue is an idiom, but in context to the song, I think she's saying that when asked to explain her illness, she can't, because it's just too painful to talk about for her, or it's hard to explain.
