its a reference to the company inspector of the buffalo creek disaster in W Virginia Feb. 1972. against his better judgment he gave the dam a green light after inspection and it collapsed a day later killing 125 people and whipped out several towns.

ive been told this song is alluding to the Buffalo Creek flood of Logan County West Virginia in 1972 which killed 125 and left 5000 people homeless. The flood was caused by the collapse of a dam that held the black waste water of a coal company.
