Ik this comment was like 1 year ago but I think he doesn't just mean physical scars, but just mental. Nikki might not have any physical scars but maybe when people saw him after his death for like a minute or 2 they didn't see him as Nikki, but as a heroin user. He probably wrote this to tell people no matter what people say to you or how you are scarred, that is not really you.
