Scarface began his career as Akshen (pronounced Action) recording solo for Lil' Troy's Short Stop Records, a local label in Houston. He would go on to sign with Rap-A-Lot Records and join a group who were collectively known as Geto Boys replacing one member who left, and released the group's second album Grip It! On That Other Level (1989), a highly successful LP that garnered the group a large fanbase, in spite of their violent lyrics keeping them from radio and MTV. He took his stage name from the 1983 film Scarface.

The album Mr. Scarface Is Back was a success, and Scarface's popularity soon overshadowed the other Geto Boys. Scarface remained in the group while releasing a series of solo albums that kept him in the public view with increasing sales. This peaked with The Diary and The Last of a Dying Breed, which received overwhelmingly positive reviews and sales, and earned him Lyricist of the Year at the 2001 Source Awards.

In 2002, he released The Fix, the follow up to The Last of a Dying Breed and returned to the studio with the Geto Boys for their album, The Foundation. He was also featured on The Biggie Duets alongside Big Gee and Akon. He guest-appeared on Ray Cash's debut single "Bumpin' my Music".

In addition to his career as a lyricist, Scarface has also been the coordinator and president of Def Jam South since 2000, where he has fostered the career of popular rapper Ludacris, whom he originally signed to the label.

Scarface has appeared on Freeway's album Free at Last and will appear on Beanie Sigel's album, The Solution. Scarface is currently planning production; he has produced three tracks on UGK's Underground Kingz: "Life In 2009," "Still Ridin' Dirty," and "Candy."

Some of Scarface's early music videos ("A Minute to Pray, A Second To Die") featured community activist Quanell X in supporting roles.

In 2008, Scarface collaborated with rapper Tech N9ne on his album Killer on the song "Pillow Talkin'".

Despite limited commercial appeal, he remains uniquely popular amongst those in the industry, and has been described as "your favorite rapper's favorite rapper". On August 6th, 2009 Scarface will be performing his final performance at the 2009 Gathering of the Juggalos. In 2005, comedian Chris Rock praised Scarface as one of the best three rappers of all time on his list of the Top-25 Hip-Hop Albums ever: "Everything Biggie did, everything 'Pac did, everything Jay-Z does was originally done on the first Scarface album. Biggie kills himself at the end of his first record; well, Scarface did it three years earlier. He was the first guy to do his rhymes in the first person about robbing people and drug dealing; he was the first guy to really talk about being depressed and being institutionalized, and how his mama is scared of him. He is the most underrated rapper of all time and absolutely in the top three. You cannot get to four without mentioning Scarface. Any rapper knows that."

This song speaks so much truth on something deep it resonates not only back in the day but even today with how I feel when I see my homies that I grew up with getting in and out of trouble and being with them during those times knowing that I could get in trouble yet ignored it but lucky for me my family and older homies and god were able to get my priorities straight and see things for what they really were learning about my history that's not taught in mainstream media and even American schools learning of The great roots of our ancestors and All that our brothers and sisters did in early America up to now especially the words and actions of Fredrick Douglass Marcus Garvey and Malcolm X and Huey P.Newton opened my eyes to the bigger picture of what we need to do as a people and myself as a person not saying I don't get discouraged once in awhile but I don't let it beat me it hurts to see my friends dying and getting locked up or doing drugs it makes me want to blowup sometimes shits fucked up.
