The only person to actually say what it's about..In case anyone hasn't figured it out from reading the lyrics, the song is about drug addiction. Specifically, the lead singers drug addiction. A reflection on how his life currently is, how no matter how hard he fights or runs, he always "falls on his face and is dragged back" into his habit. (For the time, not sure about now)The Internal Primates Forever part is not to say that he will always be using and abusing, but rather when you are hooked/addicted to drugs, you are forever a drug addict. No use and sober for no matter how many years, you're still an addict. Your mind will always think back to those times, and no matter what it's a struggle every day. So those pirmate, or monkeys on his back (aka: addiction) will be internal forever for him. Hope that helps the confused.
