This song always makes me think of when I used to live off Fulham Palace Rd in Hammersmith (London) and every day on my way to the Tube I'd walk past the hospital and there were always people smoking out the front. They weren't allowed to smoke on hospital grounds of course and the footpath along the main road is actually quite narrow, there's no room between the fence and it, so people would be virtually standing on the footpath to smoke without being in the hospital grounds. They were mainly patients from what I could see, people wearing coats over the top of hospital gowns, blue-grey legs protruding awkwardly from the bottom, and a real sense of shame and despair about them.
